credit card or electronic check by using our
On-line Application or
submit a check, money order or cashier’s check with your
application.
2)
Complete a Proof of Immunization form and submit to the
University Registrar and Admissions Office, 209 Roy Hall, Natchitoches, LA 71497. Current vaccines and boosters are
recommended but students do have the opportunity to waive
immunization for personal or health reasons. This form may
be obtained through the link at the bottom of this page.
3)
Submit past educational transcripts (high school and/or
college):

High School:

Louisiana Students graduating 2003 and after:
·
Northwestern will request an electronic copy of your transcript
from the State of
Louisiana’s
Student Transcript System. There is no need for you to
request a hard copy from your school district unless
instructed by the University Registrar and Admissions
Office.

Louisiana Students graduating 2002 and before and Out of
state/Out of Country students:
·
Please request that a copy of your final transcript be sent
directly from your school district to
Northwestern State University, University Registrar and
Admissions Office,
209 Roy Hall, Natchitoches,
LA 71497. Northwestern WILL NOT accept any transcripts from
students, parents, or ones that have not come directly from
the school.

College Students:

Please request that a copy of your final transcript be sent
directly from your previous college(s) to
Northwestern State University, University Registrar and
Admissions Office,

Submit an official copy of your
ACT or SAT
Test scores

·
Test scores are required for admission for all first-time
undergraduate students who are 24 years of age or less or
for transfer students who have less than 12 hours college
credit at another institution.

Selective Service Registration - MALES only

Male
applicants who will be between the ages of 18 and 25 by
registration of classes must submit proof of
registration with Selective Service

Male
applicants may register with Selective Service at:
www.sss.gov
6)
All students must complete the University's Proof of
Immunization form. This form can be found by following
