potential and existing small business owners with qualified, professional consultants. All
information received and developed during counseling by the SBDC staff is kept in strict
confidence.

for Counseling" Form
2. Fill in or check all applicable boxes
3. SIGN & DATE your request prior to sending it to the NSU-SBDC
4. Mail your request to: NSU-SBDC, Russell Hall, Natchitoches, LA 71497 or
fax (318) 357-6810
*It is important to note that we MUST have a signed
copy of this request in order to counsel you interactively.
*After your form is received by the NSU-SBDC, a
consultant will contact you by phone or e-mail to set up an appointment.

A completed Assets Needs Schedule
For existing businesses:
Year-end Profit & Loss statements for the last 3 years or # of
years in business or a Schedule C from tax returns.
