How to Request Services
A. EMERGENCY WORK REQUEST:
The following requests are considered emergencies and may be called
in at 4519: HEAT AND A/C OUTAGES, POWER FAILURES, STOPPED UP TOILETS,
LEAKING SHOWERS/SINKS, CRITICAL HEALTH AND SAFETY ISSUES.
B. ROUTINE MAINTENANCE REQUEST:
All requests for maintenance may be submitted on the Physical Plant
"Work Order Request" Form and routed for approval or through the
Physical Plant Web Site. To check on the status of your request
call Work Control at 4519.
C. CAMPUS PROJECT REQUEST:
Requests for campus projects to modify campus buildings must be
submitted on the "Project Request" Form and routed for approval.
The form may also be found on this web site. Project requests
may require the approval of the Facility Management
Council. (FMC) Changes in space utilization will be evaluated and
approved by the FMC.
D. HOW WE PRIORITIZE WORK REQUESTS:
- PRIORITY 1: Addresses those request that involve systems
failures or for the health, welfare, and safety of the university
community.
- PRIORITY 2: Urgent maintenance needs that does not impede
safety, but one that impaired the operation of classes or other
university operations.
- PRIORITY 3: All other routine maintenance work or
departmentally-funded request.
