Academic Suspension Appeal Form – Undergraduate Students

Northwestern State University policy allows a student with mitigating circumstances to appeal suspension from the University.

  1. Please complete page one of the form in its entirety. Page two will be completed by the college dean.
  2. In a TYPED letter, explain the basis of the appeal as well as a plan of action to improve your academic standing.
  3. Schedule a meeting with your academic advisor or department head.

Financial Aid Appeal Form – All Students

  1. Complete the Financial Aid Appeal Form found on their page.
  2. In a TYPED letter, explain why you did not make progress and a plan of action on how you will improve your academic standing.
  3. Provide any and all documentation for circumstances outside of your control.
  4. If graduating, provide a statement from your academic advisor.
